confidentialThe practice complies with Data Protection and Access to Medical Records legislation. Identifiable information about you will be shared with others in the following circumstances:

  • To provide further medical treatment for you e.g. from district nurses and hospital services.
  • To help you get other services e.g. from the social work department. This requires your consent.
  • When we have a duty to others e.g. in child protection cases anonymised patient information will also be used at local and national level to help the Health Board and Government plan services e.g. for diabetic care.

If you do not wish anonymous information about you to be used in such a way, please let us know.

Reception and administration staff require access to your medical records in order to do their jobs. These members of staff are bound by the same rules of confidentiality as the medical staff.

This practice is a member of the HeeleyPlus Primary Care Network of practices. HeeleyPlus is made up of the member practices: Gleadless Medical Centre, Sharrow Lane Medical Centre, Sloan Medical Centre, Mathews Practice (Belgrave), Veritas Medical Centre, Carrfield Medical Centre, and Heeley Green Surgery. It is also supported by Heeley Trust Social Prescribing Team for the purposes of coordinated and providing care to our patients.

Your medical centre shall from time to time and for purposes directly related to your medical and social care, share information with the management team of the HeeleyPlus PCN and with the Heeley Trust Social Prescribing Team. HeeleyPlus PCN may use some information to evaluate the effectiveness of the services being provided by these organisations. No practice shall share patient information with any other member practice without explicit consent. Please inform reception if you are concerned about any element of this and your confidentiality and they will arrange for the Practice Manager to speak with you.
